Referral Process
Referrals to our clinic
are made by physicians, insurance company representatives or patients
themselves. Persons who are referred by a physician should bring a
prescription from the physician. Persons under the age of 18 should be
accompanied on the initial visit by a parent or guardian.
On your first visit, you
will be asked to complete a Patient Registration Form. Information
requested will include diagnosis, date of injury (if applicable) and other
pertinent medical information. Please bring your insurance card or other
verification of insurance coverage with you. Financial responsibility for
services is ultimately yours if your carrier does not approve insurance
coverage.
